Domestic Clients
For all domestic orders, the test requisition form (included in test kit) must be signed by a medical practitioner and accompany all specimens sent to the Great Plains Laboratory. Alternatively, practitioners can submit signed kit requests to our lab via fax or through our online test requisition form. The Great Plains Laboratory does not offer testing in New York State.
International Clients
Due to recurrent problems with postal services in different countries, The Great Plains Laboratory, Inc. (GPL) will start sending test results only via e-mail to physicians and patients outside the United States on September 2nd, 2008. Please note that printed test results and hard copies of invoices will no longer be sent via regular mail.
Hard copies of invoices will be sent via regular mail upon request only. Please make sure you write clearly the e-mail address where you want your test results sent on your test requisition form.
