Payment is required with specimen for your test to be processed.

You can either take advantage of our cash discount by sending payment in full with your sample, or enclose a 20% prepayment of the insurance cost and we will file a claim to your insurance company. Please note: If you are planning to send the 20% insurance prepayment, please check your test below to make sure it is one we can bill your insurance for directly. The new price list is in US dollars. The prices below also apply to all the international prices.
